Position Summary:
- The Wellness Coordinator is responsible for assisting Neighborhood Family Practice patients and Near West Side residents with planning and managing a variety of health promotion activities to meet their needs. The Wellness Coordinator must develop services that are part of the comprehensive clinical and community health program that is the mission of NFP.
