Job Title: Access Plan & Technology Coordinator
Reports To: Executive Director
FLSA Status: Exempt
Summary: Responsible for coordinating and monitoring the Cuyahoga Health Access Partnership’s (CHAP) Access Plan for low-income, uninsured adults. Also manages CHAP’s technology infrastructure. Duties and Responsibilities include the following. Other duties may be assigned.
Access Plan
- Implement Access Plan Model electronically and pursue enhancements to the business plan model to further CHAP’s mission of providing access to primary and specialty care.
- Manage daily operations of the Access Plan to ensure seamless experience for clients. Serve as CHAP resource for client questions via telephone, web, or in person.
- Serve as a first point of contact for partner staff or clients who encounter problems while using the Access Plan.
- Facilitate communication and coordination among CHAP partners, foundations and governmental entities through the Program Development Committee.
- Establish goals and deadlines for Access Plan implementation at additional provider sites according to the Access Plan Model.
- Assist the Executive Director with foundation and grant reports, administration, and applications.
- Manage the development of the utilization and tracking component of the Access Plan Model.
- Oversee the development of the provider service directory and on-going updates required.
- Monitor and oversee compliance with the Access Plan Model eligibility and enrollment procedures.
- Collaborate with the Administration & Communications Coordinator to maintain the CHAP website and any print or electronic communications materials.
Technology
- Manage CHAP’s internal information systems, including on-going support for PCs, data and information sharing, and email communications.
- Serve as first-line support for CHAP-related hardware and software issues for internal staff and FACTS/ImageAR users.
- Perform periodic software updates, system back-ups, virus protection updates, and monitor overall system performance for CHAP employees.

Education/Experience
- Bachelor’s Degree in a related field required.
- Experience with financial counseling or patient enrollment in health settings such as hospitals, health clinics or insurance plans a plus.
- Understanding of healthcare charity care programs and eligibility processes within hospital and/or clinic settings preferred.
- Proficient in Microsoft Word, Excel, and PowerPoint and various publishing software programs. Able to learn new software programs and work in databases.
- In-depth knowledge of PC-specific hardware configurations and software systems needed.
- Understanding of general database functions preferred.
- Exceptional time management skills and the ability to balance multiple projects simultaneously.
- Able to communicate effectively (written and oral) with diverse stakeholders.

About CHAP
- CHAP is a collaboration of public and private organizations that have formed a countywide partnership to provide a coordinated system of healthcare access for Cuyahoga County’s low-income, uninsured adults. The stand-alone organization was incorporated in the State of Ohio in September 2009. It was founded on the principle that all stakeholders have a shared responsibility to address the uninsured crisis in Cuyahoga County.
Our Goal
- CHAP’s vision is to provide a system of access for uninsured adults in Cuyahoga County. While CHAP focuses on the importance of primary care, its efforts will extend throughout the entire spectrum of care.
Our Partnerships
- CHAP is unique because of the extent of the institutional commitments of its partners to caring for the low-income, uninsured of Cuyahoga County. It is managed by a Board of Directors that is comprised of all initial stakeholders, foundations and government entities. To demonstrate each organization’s commitment to this vital work, each founding member of CHAP has signed a Memorandum of Understanding.
- All participating stakeholders acknowledge the importance of working together to improve the health and wellbeing of our region. Each of the member organizations are committed to serving our community by caring for the uninsured and working toward the common goal of increasing healthcare access.
Our Access Plan
- CHAP’s first major initiative will be to establish an access plan that connects participants to a primary care site or “medical home.” This will help patients manage their health more effectively, and in turn, improve health outcomes.
- The access plan will improve efficiency across the healthcare system by promoting care coordination and reducing episodic care, duplicative care and unnecessary evaluations.
- Initially, the access plan will be open to all uninsured adults at or below 200 percent of poverty. Program enrollment is limited to Cuyahoga County residents between the ages of 19 and 64 who do not qualify for public healthcare programs (Medicare, Medicaid and Veterans’ health coverage/VA) and are without an employer-sponsored insurance option.
