POSITION TITLE: Wellness Coordinator

Position Summary:

• The Wellness Coordinator is responsible for assisting Neighborhood Family Practice patients and Near West Side residents with planning and managing a variety of health promotion activities to meet their needs. The Wellness Coordinator must develop services that are part of the comprehensive clinical and community health program that is the mission of NFP.
• Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:
  1. Meets regularly with clinical staff to identify barriers to healthy lifestyles in the community.
  2. Coordinates with management and clinical team to establish short and long range goals and objectives, set priorities, plan methods to be used to address these problems, and evaluate progress.
  3. Participates in community outreach activities for NFP including health fairs, community-wide events and educational presentations.
  4. Provide individual and small group wellness education and counseling to patients of NFP who have specific lifestyle-related health problems.
  5. Develop healthy eating and physical activity plans for patients and their families.
  6. Plan and provide wellness education and interventions at a variety of community sites within NFP guidelines (re: mission, funding, staff support, etc.). This includes ongoing classes or support groups, as well as one-time presentations.
  7. Work with existing neighborhood groups to plan healthy lifestyle interventions that will be part of their community health programs.
  8. Coordinates NFP’s transportation assistance program.
  9. Perform all other tasks related to wellness as assigned by the Medical Director, Executive Director and the Supervisor of the position.

SKILLS REQUIRED

• Working knowledge of the local community.
• Experience in Public Health is preferred.
• Must possess good communication skills.
• Must have his/her own transportation.
• Ability to communicate in English and Spanish desired.

• Previous experience that is necessary background to qualify for this position:
• Bachelor’s degree in a health related field.
• Experience working with individuals on wellness education and counseling is essential
• Computer experience is essential.
